To facilitate the application/registration processes for both the Qualification and Certification of Non-Destructive Testing (NDT) Personnel and Exposure Device Operators (EDO), the following documents are available in HTML and Portable Document Format (PDF).
The HTML versions of the documents are not forms; they display the information as found on the forms for viewing purposes only and will not be accepted if used to apply with the NDT Certifying Agency.
If you wish to submit a form, you must print only the PDF version, complete the form and mail or fax it to the NDT Certifying Agency. To view/print the PDF version, you will need Adobe Acrobat Reader 4.0 (or later) on your system.
